Advance directives are legal documents that allow you to convey your decisions about end-of-life care ahead of time. They provide a way for you to communicate your wishes to family, friends and health care professionals, and to avoid confusion later on.

Your personal and medical information is confidential. Medical records and communications about your condition will be kept private.
Your physician must clearly explain the advantages and risks of any procedures, tests, or treatments. You must give your permission for such care and have the right to refuse any treatment.
You will be notified about the frequency of the services that are proposed to be furnished, and to know the names and responsibilities of the disciplines providing your care. You also have the right to receive information about the services covered under the hospice benefit and about the scope of services that we will provide and specific limitations on those services.
